Grade 4 Gynecomastia with Skin Excision Case Study

One of the most important conversations we have with patients is about expectations. For mild to moderate cases, a completely flat, almost invisible result is very achievable. But for Grade 4 gynecomastia on a larger frame, the starting point is much more severe, and the goal shifts slightly. The aim is not perfection. It is a dramatic, meaningful improvement that changes the way the chest looks and how clothing fits.

This patient had Grade 4 gynecomastia with a full, rounded chest that showed through clothing from every angle. After surgery that included gland excision, liposuction, and skin removal, his chest sits much flatter and more proportional to the rest of his body.

Where This Case Started

The before photos on this patient show a level of severity that goes well beyond simple fullness. From the side, the chest tissue hangs forward and downward in a heavy, pendulous shape. The areola has drifted down to the bottom of the tissue mound and is pointing toward the ground rather than facing forward.

From the front, both sides of the chest are very full and rounded with a shape that closely resembles a female breast. The tissue covers the entire pectoral area, and there is no visible muscle definition or flat surface anywhere on the chest. The overall look is dominated by the volume and weight of the tissue.

This level of enlargement involves a large amount of glandular tissue, a significant layer of fat, and skin that has been stretched well beyond its natural resting point. All three of those issues had to be addressed during the procedure.

The Three-Part Surgical Plan

Our team used a three-part approach to match the severity of this case:

• Gland excision: The dense glandular tissue behind each areola was removed to eliminate the root cause of the enlargement. In a Grade 4 case, the volume of glandular tissue is typically much larger than what you see in lower grades.
• Liposuction: The fatty tissue across the chest was removed to bring down the overall volume. On a larger frame with this much tissue, the lipo portion of the procedure is essential for reshaping the full contour of the chest.
• Skin excision: After the gland and fat were removed, the stretched-out skin could not tighten down enough on its own. Our team removed the excess skin directly to create a flatter, tighter result. Without this step, the chest would be smaller but the skin would hang loose over the reduced volume.

Skin excision is not needed in every case. It is typically reserved for more severe gynecomastia grades where the skin has been stretched beyond its ability to retract naturally. For this patient, it was a necessary part of the plan.

What the After Photos Show

The after photos show a chest that looks completely different. From the side, the heavy, hanging shape is gone. The chest sits much closer to the body, and the tissue no longer pulls downward. The areola has moved back to a more natural position and faces forward instead of pointing down.

From the front, the overall volume has been reduced dramatically. The rounded, breast-like shape is replaced by a flatter, more masculine contour that fits better with the patient’s frame. There is still some fullness present, which is normal for a larger-framed patient, but the shape is completely different from where things started.

A visible incision line runs along the lower chest from the skin excision. That scar will continue to fade over the coming months and is a normal trade-off in severe cases where skin removal is necessary to get a good result.

Setting the Right Expectations

Grade 4 cases on a larger frame will not produce the same result as a Grade 1 case on a lean body. That is just reality. But the improvement can still be massive. The difference between a chest that hangs and sags versus one that sits flat and fits inside a shirt normally is life-changing for most patients, even if the result is not magazine-cover perfect.

Why Severe Cases Need a Complete Plan

Grade 4 gynecomastia cannot be fixed with one technique alone. The gland has to come out. The fat has to be reduced. And in many cases, the excess skin has to be removed. Skipping any one of those steps will leave the patient with an incomplete result.
