Case Study: Grade 1 Gynecomastia Before and After

When everything lines up the right way, gynecomastia surgery can produce a result that looks like the tissue was never there in the first place. The grade is mild, the skin is healthy, the body type is proportional, and the surgical plan is straightforward. That is exactly what happened with this case.

This patient has an average build with Grade 1 gynecomastia. The amount of tissue involved is small, but the impact on the shape of the chest is easy to see in the before photos. After surgery, the profile is completely flat and the front view confirms a clean, well-shaped chest from every angle.

Small Amount of Tissue, Big Visual Impact

Grade 1 is the mildest form of gynecomastia, but “mild” does not mean “invisible.” Even a small amount of glandular tissue behind the areola can change the way the chest looks, especially on an average frame where there is not a lot of extra body fat to hide it.

The issue with Grade 1 is that the tissue tends to sit right at the surface. It pushes the areola outward and creates a soft, rounded bump that shows through thin shirts, catches light differently than the rest of the chest, and stands out when the arms are raised or the body is turned to the side.

This patient’s case follows that pattern exactly. The tissue is not large, but it is in a spot where it is very hard to miss.

The Before Photos

The side view is where Grade 1 gynecomastia really reveals itself. This patient’s chest has a noticeable bump that pushes outward from the body right at the level of the areola. The rest of the torso is relatively flat and proportional, which makes that bump stand out even more. It breaks the clean line from the collarbone down and creates a small but obvious point of projection.

The front view shows a softer, rounder shape across both sides of the chest. The pectoral area lacks the firm, flat look that would normally match this body type. Instead, there is a gentle puffiness centered around each areola that gives the chest a slightly swollen appearance.

What Our Team Did

Our team used the standard approach for this Grade 1 case:

• Gland excision: The glandular tissue behind each areola was removed directly. Even though the volume was small, the gland was the entire source of the problem. Taking it out is the only way to flatten the chest and prevent the issue from returning.
• Liposuction: A small amount of surrounding fatty tissue was removed to blend the treated area into the rest of the chest. This step makes the final result look smooth and even instead of scooped out or uneven around the areola.

For a Grade 1 patient with good skin quality on an average frame, this is a quick, clean procedure. The tissue volume is low, the skin does not need help with retraction, and the recovery timeline is typically shorter than higher-grade cases.

The After Photos

The after photos show exactly what a strong Grade 1 correction should look like. From the side, the bump is completely gone. The chest follows a flat, straight line from top to bottom with no tissue pushing outward at any point. The profile is clean and tight.

From the front, the puffiness around each areola is gone. The chest has a firmer, more angular shape that fits naturally with the rest of the body. The pectoral contour is well-defined, and the areolas lay flat against the chest without any swelling or protrusion underneath them.

There are no visible signs of the procedure from either angle. The incisions are hidden, the contour is smooth, and the chest looks natural. This is the kind of result that sets the standard for what Grade 1 corrections can achieve.

What Makes a Great Result Possible

Not every case will look like this one. The outcome depends on the grade, the tissue type, the patient’s skin quality, and the skill of the surgical team. But when the conditions are right, gynecomastia surgery can deliver a result that is essentially invisible.

Grade 1 cases on average-build patients tend to be the best candidates for this kind of outcome. The tissue removal is minimal, the skin bounces back quickly, and the final shape is easy to control because there is not a lot of volume to manage.
