Most before-and-after photos are taken within the first few months of surgery. That gives a good idea of the early improvement, but it does not show the full picture. Gynecomastia results continue to change and improve over time as swelling goes down, skin tightens, and the body fully heals.

Why Long-Term Photos Matter
In the weeks and months after gynecomastia surgery, the chest goes through a lot of changes. There is swelling that can make the chest look fuller than the final result. There is bruising that fades. What the Before Photo Shows
The before photo shows Grade 4 gynecomastia with significant volume and skin laxity across both sides of the chest. The tissue is heavy and rounded with a breast-like shape that extends well past the normal male chest line. The areolas are large and have been stretched and pulled downward by the weight of the tissue underneath.
There is also visible skin laxity in the before photo. The skin has been stretched by the tissue for a long time and has lost much of its natural elasticity. That kind of skin quality is an important factor in the surgical plan because it tells our team that the skin will not be able to tighten down on its own after the tissue is removed.
How Our Team Treated This Case
Our team used a two-part approach for this patient:
- Liposuction: The fatty tissue across the chest was removed to bring down the overall volume and reshape the contour. For a larger-framed patient with this much fullness, liposuction is responsible for the bulk of the volume reduction.
- Skin excision: The excess, stretched-out skin was removed directly to create a tighter, flatter result. Without this step, the chest would have been smaller in volume but the loose skin would have draped over the reduced tissue and left the chest looking deflated rather than flat.
The combination of volume reduction and skin removal is what allows the chest to look both smaller and tighter at the same time.
What the Two-Year Result Looks Like
The after photo shows a dramatically different chest. The heavy, rounded shape is gone and the chest now sits much flatter against the body. The overall contour is more masculine and more proportional to the rest of the patient’s frame.
The areolas are smaller and sit in a more natural position on the chest. They face forward rather than being pulled downward, and they lay flat against the skin without any tissue mounding behind them. That change alone makes a major difference in the overall appearance.
The skin looks smooth and well-settled. At two years, any scars from the skin excision have had time to fade and mature. There is still some fullness present, which is expected and natural on a larger frame, but the shape of the chest no longer resembles what it looked like before surgery.
