Your Pre-Op Timeline

Although each gynecomastia surgery case is unique, there’s a general timeline you can anticipate. This timeline is pretty consistent across various types of gynecomastia surgeries you might undergo, including:

The weeks leading up to surgery follow a pretty predictable pattern. Here’s a general timeline of what needs to happen and when:

2-4 Weeks Before
  • Complete any required lab work or medical clearances
  • Stop smoking and adjust medications as directed
  • Purchase compression garment and recovery supplies
1 Week Before
  • Fill prescriptions that our office provided
  • Prepare your recovery space at home
  • Confirm transportation and caregiver arrangements
Day Before
  • Use antibacterial soap during your showers
  • Prepare easy meals for the first few days of recovery
  • Stop eating & drinking after midnight
Morning of Surgery
  • Shower again with antibacterial soap
  • Wear comfortable, loose clothing to your surgery
  • Arrive at the scheduled time with any required items

Lifestyle Adjustments Before Surgery

Certain habits and substances interfere with surgery and healing, so you’ll need to make some temporary changes in the weeks leading up to your procedure. Here’s what to stop and what to keep doing.

STOP These
  • Smoking and Vaping: Stop all nicotine products at least 4 weeks before surgery. Nicotine restricts blood flow.
  • Alcohol: Avoid alcohol for at least one week before surgery. It thins your blood and increases bleeding risk.
  • Blood Thinners: Stop aspirin, ibuprofen, naproxen, and similar medications 7-10 days before unless told otherwise.
  • Supplements: Discontinue fish oil, vitamin E, garlic supplements, and other herbal products that thin blood.

  • Prescription Medications: Keep taking your regular medications unless our office specifically tells you to stop.
  • Healthy Eating: Maintain a balanced diet with plenty of protein to support healing after surgery.
  • Light Exercise: Stay active with walking and light movement, but avoid intense workouts the week before.
  • Good Sleep: Prioritize rest and manage stress levels leading up to your procedure.

Arrival and Check-In

You’ll arrive at the surgical facility about 30-60 minutes before your scheduled surgery time. This window allows time for paperwork, changing into a gown, and getting everything started before one of our doctors is ready for you.

Registration

The front desk will check you in and verify your information. They’ll collect your ID and insurance card, have you sign consent forms, and confirm your emergency contact information. This usually takes 10-15 minutes.

Your driver can wait in the waiting area or leave and come back when you call later. Either way works, though having them stay makes pickup easier since you’ll be ready to leave fairly quickly after waking up.

Pre-Op Area

A nurse will bring you to the pre operative area, where you will change into a surgical gown and get settled. Your vital signs will be checked, and your medical information will be reviewed carefully.

Depending on the type of anesthesia planned for your procedure, the anesthesia team may also speak with you before surgery to explain what to expect and ensure you feel comfortable and informed.

During Your Surgery: What Happens in the Operating Room

Once you are in the operating room and anesthesia has been administered, your procedure will begin. Gynecomastia surgery typically takes about one to two hours, though the exact timing varies based on your case.

Step 1: Anesthesia and Positioning

The anesthesia team ensures you remain comfortable and safe throughout the procedure, with continuous monitoring of your vital signs. At the same time, the surgical team positions you properly and prepares your chest with a sterile solution to reduce the risk of infection.

Step 2: Making the Incisions

Dr. Okoro or Dr. Lee will make small incisions along the lower border of your areola where they’ll blend naturally with the surrounding tissue. The placement and length of these incisions depend on how much tissue needs removal and your specific anatomy.

Step 3: Removing Glandular Tissue

Our surgeons carefully separate and remove the firm breast gland tissue from your chest muscle and surrounding structures. This step directly addresses the dense tissue responsible for chest fullness and creates a flatter, more defined contour.

Step 4: Eliminating Excess Fat

Liposuction removes fatty deposits from your chest and surrounding areas to create better definition and contour. A thin cannula moves beneath your skin to break up and suction out unwanted fat cells that contribute to the enlarged appearance.

Step 5: Sculpting and Contouring

Dr. Okoro or Dr. Lee carefully shape the remaining tissue to create a natural, masculine chest appearance that looks proportionate to your body. They check symmetry between both sides and makes adjustments to ensure balanced, aesthetically pleasing results.

Step 6: Closing the Incisions

The incisions are closed with sutures that will either dissolve on their own or be removed at a follow-up appointment. A compression garment is placed immediately over your chest to support healing, minimize swelling, and help your skin conform to its new shape.

Waking Up in Recovery

After your procedure is complete, you will be moved to the recovery area. How you feel at this stage depends in part on the type of anesthesia used. Patients who receive general anesthesia may feel groggy or disoriented at first. Those who have IV sedation or local anesthesia often feel more alert sooner.

A nurse will monitor your vital signs closely and make sure you are stable and comfortable. Some chest soreness is normal and will be managed with appropriate medication. If you feel cold, warm blankets are provided.

Your compression garment will already be in place. It should feel snug and supportive. That pressure helps control swelling and support your chest contour, so it is important not to adjust it.

Most patients remain in recovery for about 30 to 60 minutes. Before discharge, the team confirms your pain is controlled, there is no concerning bleeding, and you are steady on your feet. You will receive written instructions, any necessary prescriptions, and direct contact information for questions after you return home.
